New Patient Registration
We accept patients living in Freemantle, Shirley, Millbrook, Lordshill or Coxford - call or ask at reception for the exact catchment area if you are unsure.
If you live within our practice catchment area and you would like to register with us, please come to the surgery with your signed NHS medical card or complete the registration forms available at reception. The forms are also accessible to complete online below, though you will still need to visit the surgery to complete your registration.
For entitlement to register with a GP practice you must provide:
- Evidence of identity if you are a UK resident or evidence of status for patients from overseas including the European Economic Area.
- Further details are available from reception.
Online 'Pre-Registration' With The Practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct, as well as provide evidence to confirm who you are.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. If you complete the form online, when you come to the surgery you will be asked to sign a copy of your form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
If you fall ill while away from home, or if you are not registered with a GP practice but you need to see one, you can still contact us to ask for treatment. You can receive emergency treatment for up to 14 days. After that you will have to register either as a temporary resident or permanent patient. The Shirley Health Partnership does not have to accept you as a temporary resident although we do have an obligation to offer emergency treatment.
Consider calling NHS 111 if you urgently need medical help or advice but it's not a life-threatening situation. You can also call NHS 111 if you're not sure which NHS service you need.
Registration as a temporary resident allows you to be taken on to our practice’s list for up to three months. If you are registered with another practice but are away from your home area, you can register temporarily with a practice near where you are currently staying and still remain a patient of your registered practice. You cannot register as a temporary resident if you are already signed up with another local practice, although you can consider signing up with us as a permanent patient.
Please try to have the following information available when you attend your appointment for the first time:
- Contact details of your registered or previous practice
- Details of your ongoing medical problems
- Details of medical problems you have suffered in the past
- The name and dose of any medicines that you are currently taking
- Details of any allergies
